Job Description
El Hogar Community Services, Inc. operates outpatient mental health, substance abuse and prevention programs utilizing the Psychosocial Rehabilitation Model serving adults in the Sacramento area who are experiencing mental illness.
Summary: The Enhanced Care Management Care Manager is responsible for delivering support services to clients referred to or attending El Hogar’s programs. The Enhanced Care Management Care Manager provides services with the principles of the Strengths Based and Psychosocial Rehabilitation Model which may include but are not limited to: Assessments, service plan development, case management, skills training, group facilitation, linkage to resources, and crisis intervention. The Enhanced Care Management Care Manager carries a caseload of up to 60 Managed Care Plan members and maintains contact as defined by the Program Contract(s).
Essential Duties and Responsibilities: include the following. Other duties may be assigned;
- Maintain a caseload of approximately 40-60 Managed Care Plan (MCP) Members
- Offer services where the Managed Care Plan (MCP) Member lives, seeks care, or finds most easily accessible and within MCP guidelines
- Connect MCP Members to other social services and supports he/she may need
- Advocate on behalf of MCP Members with health care professionals
- Use motivational interviewing, trauma-informed care, and harm-reduction practices
- Work with hospital staff on discharge plan
- Engage eligible MCP Members
- Accompany MCP Member to office visits, as needed and according to MCP guidelines
- Monitor treatment adherence (including medication)
- Provide health promotion and self-management training
- Arrange transportation
- Contact MCP Member to schedule in-person visit with the care coordinator
- Prepares assessments, progress notes, and other clinical documents;
- Conducts assessments, service plan development, case management, psychosocial rehabilitation, crisis intervention, program transfers, and discharges;
- Creates and implements group counseling and crisis intervention with consumers;
- Develops and facilitates psychosocial rehabilitation and co-occurring skill groups;
- Carries a consumer case load, and has responsibility for maintaining assigned productivity levels each month;
- Actively contributes in a positive manner during El Hogar provided clinical supervision.
Supervisory Responsibilities:
This position has no supervisory responsibilities.
Qualifications:

Education and/or Experience: Minimum qualifications can be met in the following ways:
High school diploma or equivalent (required)
AND
- Four years of full time direct care experience in a mental health related field;
OR
- Two years of full time direct care experience in a mental health related field; and two (2) years of education (60 semester units or 90 quarter units) with a minimum of 12 semester (18 quarter) units in mental health related subject area such as child development, social work, human behavior, rehabilitation, psychology, or alcohol and drug counseling;
- Experience working with older adults is preferred.
- Computer literacy in Word and Excel
