FTE: 0.800000 Shift: 1 Schedule: 7:30am-4pm(ish) working 4days a week, No nights, weekends or holidays Job Summary: This positionis an outpatient role that job shares between the Cancer Center and HomeCare. The Social Worker is responsible culturally responsivepsychosocial screening and assessment to identify social, psychological,behavioral, economic, and environmental needs of outpatients and theirfamilies. The Social Worker establishes a plan of care that providesmaximum benefit to patients and their families. The goal of social workservices is to be an advocate for patients across the continuum,connecting them with the necessary resources to ensure safe andeffective care. EXPERIENCE DESCRIPTION: A minimum of 1-2 years ofexperience in an acute care setting is preferred. EDUCATION DESCRIPTION:Master\'s Degree in Social Work is required. SPECIAL SKILLS DESCRIPTION:Demonstrated ability to work effectively in a team environment andinteract with physicians, other professional disciplines, externalagencies and organizations. Requires strong computer skills. Medicalsocial work, including discharge planning preferred. Interpersonalskills necessary to obtain information from patients/ families, andinteract effectively with patients, families and hospital personnel.Analytical skills necessary to assess patient\'s needs, developassociated discharge planning and develop solutions to problemsconcerning patient\'s mental and physical well-being. LICENSUREDESCRIPTION: Current State of Wisconsin license in social work isrequired.
