Neighborhood Family Practice was founded in 1980 in response to a concern that residents of Cleveland’s near west side lacked access to primary health care. NFP focuses on providing high quality primary care in a medical home setting. Our mission is to partner with the community for everyone’s best health.
As a community provider, we’ve established medical offices in convenient neighborhood locations - Ridge Road, Tremont, Detroit Shoreway and Bellaire-Puritas. Our service area spans 12 neighborhoods across Cleveland’s west side and is comprised of a very diverse mix of residents. NFP serves one of the largest Hispanic communities in Ohio and provides care to rising number of refugees.
