Social & Community Service Coordinator

Develop and implement strategies to improve the health of individuals with developmental disabilities, and their communities. Coordinate programs and materials about health issues faced by the community. Collect data and discuss health concerns with members of specific populations or communities. Evaluate the effectiveness of an organizations social service programs and educational materials. Maintain individuals records according to HIPPA, state, federal laws and guidelines. Adjust existing programs or events to better suit the needs of the community. Educate about the availability of healthcare services. Provide a link between the community and healthcare professionals. Collect and analyze data to learn about communities to identify health-related issues, collect data, and discuss health concerns with program participants to improve programs and services. Provide informal counseling and social support. Help obtain funding and other resources. Advocate for individual and community needs.

Requirements: Bachelors Degree in social work, sociology, psychology, education, and 6 months of experience.
