Job Description


The Case Manager coordinates all services necessary to maximize consumer recovery and independence for individuals with mental illness including implementing strategies for obtaining services and supports that are goal-oriented and individualized. This opportunity is located at our Saginaw Crisis Residential Unit. The work hours for this position are 24 hours per week, maybe more on occasion. 1st/2nd shift, will include some weekends . The program is intended to be a short-term alternative for individuals at risk of admission into a psychiatric inpatient hospital but who can be appropriately served in a setting less intensive than a hospital.

How you will change lives

Case Management

  • Ensure consumer involvement in the Person Centered Planning Process (PCP).
  • Facilitate the implementation of Individual Plan of Service.
  • Perform comprehensive, ongoing psychosocial assessments.
  • Participate in legal proceedings including petitioning and testifying in clinical emergencies.
  • Meet with assigned persons-served face-to-face as deemed medically necessary.

Coordination of Care

  • Facilitate the coordination of consumer intakes, discharges, and transfers.
  • Communicate with all service providers in Treatment Plan.
  • Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.
  • Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
  • Provide referral to appropriate community resources.

We are Hopeful you have

  • Bachelor's degree in a human services (Social Work, Psychology, Sociology, Criminal Justice ) field from an accredited institution required.
  • Prior case management experience in a mental health or rehabilitation setting preferred.
  • Valid Michigan Driver's License.
